Job summary

Heritage Health Network in Palm Desert, California, invites you to join as Lead Care Manager for the Enhanced Care Management program. This hybrid role combines remote coordination with in-field visits to build strong relationships with members across the Low Desert region.

You will lead care planning, coordinate with providers, educate members, and ensure documentation is timely and accurate. Travel within Orange County is expected; bilingual English/Spanish is preferred to serve diverse

Qualifications

• Experience in care or case management, community-based preferred.
• Knowledge of Medi-Cal and ECM strongly preferred.
• Excellent communication and interpersonal skills.
• Team-focused with ability to travel within Orange County.

This is a hybrid position (2 days remote, 3 days in the field) , giving you the flexibility to work from home while building strong, in-person connections with members across Low Desert regions.

Key Responsibilities
