Job summary

United Health Centers of the San Joaquin Valley is hiring a Lead Care Manager to oversee ECM patient care across the team. You will act as the primary liaison for patients, families, and care providers, conducting assessments and developing comprehensive care plans to address gaps in treatment and services.

You will manage a caseload, coordinate services, and advocate for patients while ensuring timely follow-up, referrals, and hospital discharge coordination.

Qualifications

• LVN license or MA certification or bachelor’s degree or progressive experience required.
• High school diploma or GED required.

Responsibilities

• Serve as primary contact for ECM-enrolled patients and care team.
• Manage full patient caseload per guidelines.
• Conduct Comprehensive Assessment and develop Care Management Plan.
• Coordinate with patient/family and care team to ensure seamless care.
• Provide outreach and engagement; monitor plan adherence.
• Coordinate discharge planning and hospital follow-up.
• Attend required ECM training and perform other duties as assigned.
