Job summary

Newave Care is seeking an Attending Nurse Practitioner to provide comprehensive medical management for residents in long-term care, ventilator units, and post-acute rehab settings. The NP will round regularly, coordinate care with interdisciplinary teams, and apply evidence-based interventions to improve outcomes and reduce hospitalizations.

Qualifications

• Board certification as AGNP, GNP, or FNP.
• Active, unrestricted NP license in state of practice.
• Current BLS and ACLS certifications.
• Prior experience in long-term care, subacute rehab, ventilator care, or chronic disease management preferred.

Responsibilities

• Provide medical management for patients in long-term care, ventilator, behavioral health, and subacute settings.
• Regular rounding to ensure continuity of care and timely follow-ups.
• Manage chronic/complex conditions with evidence-based interventions and episodic care as needed.
• Prevent avoidable hospitalizations through early intervention and vigilant surveillance.
• Coordinate with nursing, rehabilitation, and physicians to optimize care plans.
