Job summary

Newave Care is seeking an Attending Physician to deliver comprehensive medical care across long-term care, subacute/post-acute, ventilator, and behavioral health units. You will round regularly, lead multidisciplinary teams, and implement evidence-based interventions to improve outcomes and reduce avoidable hospitalizations.

The role emphasizes proactive, patient-centered care, strong clinical judgment, and collaboration with nursing staff and NPs/APPs to optimize care plans and goals of care

Qualifications

• MD/DO with board certification in Internal Medicine, Family Medicine, or Geriatrics.
• Active state license and DEA registration.
• Experience in post-acute/long-term care or behavioral health.
• Proficient in EMR and clinical documentation.

Responsibilities

• Provide direct medical care to residents in long-term, post-acute, ventilator, and behavioral health units.
• Routinely round, evaluate, and follow up to ensure continuity of care and early intervention.
• Lead multidisciplinary care team meetings and contribute to individualized care plans.
• Manage chronic and complex conditions with timely interventions and therapies.
• Collaborate with NPs/APPs, nursing, rehab, and admin staff to coordinate care.
• Discuss goals of care and palliative options with patients and families.
