Job summary

Newave Care is seeking an Attending Physician to deliver comprehensive medical care for residents in long-term care, subacute/post-acute, ventilator, and behavioral health units. You will round regularly, lead multidisciplinary teams, and implement evidence-based interventions to improve outcomes and reduce hospitalizations.

The role emphasizes goal-concordant care, clear communication with families, and robust documentation in the EMR.

Qualifications

• MD or DO degree required and board eligibility in Internal Medicine, Family Medicine, or Geriatrics.
• Active, unrestricted medical license in the appropriate state.
• Experience in post-acute/long-term care and behavioral health a plus.

Responsibilities

• Provide direct medical care to residents across long-term, subacute, ventilator, and behavioral health units.
• Conduct rounds, evaluations, and timely follow-ups to ensure continuity of care.
• Lead multidisciplinary care team meetings and contribute to individualized care plans.
• Manage chronic and complex conditions with appropriate interventions and therapies.
• Communicate clearly with nursing staff, NPs/APPs, patients, families, and consultants.
• Coordinate care with nursing, rehabilitation, leadership, and NP.
• Support goals of care discussions and palliative options with patients and families.
• Promote preventive care and proactive treatment plans for healthy aging.
• Document accurately in the EMR and maintain strong clinical documentation.
• Develop proactive strategies to reduce avoidable hospital transfers and readmissions.
