Job summary

Newave Care seeks an Attending Physician to deliver comprehensive medical care across long-term, subacute, ventilator, and behavioral health units. You will round regularly, lead multidisciplinary teams, and implement decisive, evidence-based interventions to improve outcomes and reduce hospitalizations.

The role requires board certification in Internal Medicine, Family Medicine, or Geriatrics, an active NY/NJ license, and experience in post-acute or long-term care.

Qualifications

• MD or DO from an accredited medical school.
• Board Certified/Board Eligible in Internal Medicine, Family Medicine, or Geriatrics.
• Active, unrestricted state medical license and current DEA registration.
• Experience in post-acute/long-term care, ventilator care, or behavioral health preferred.
• Proficiency with EMR systems and clinical documentation.
• Demonstrated ability to collaborate with NPs/APPs, nursing, rehabilitation, and administrative staff.
• Current BLS/ACLS (or willingness to obtain).

Responsibilities

• Provide direct, comprehensive medical care to residents across long-term, subacute/post-acute, ventilator, and behavioral health settings.
• Conduct routine rounding and evaluations, and ensure timely follow-ups for continuity of care.
• Lead multidisciplinary care team meetings and contribute to individualized care plans.
• Manage chronic and complex conditions promptly and use evidence-based interventions.
• Communicate clearly with nursing staff, patients, families, and providers.
• Collaborate with facility teams to coordinate and optimize care.
• Discuss goals of care and palliative options with patients and families.
• Promote preventive care and proactive treatment plans.
• Document accurately in the EMR and maintain strong clinical documentation habits.
• Develop strategies to reduce avoidable hospital transfers and readmissions.
