Job summary

Newave Care in Illinois seeks a mission-driven Attending Physician to deliver comprehensive medical care across long-term care, subacute/post-acute, ventilator, and behavioral health units. Qualifications

• MD or DO from an accredited medical school.
• Board Certification/Eligibility in Internal Medicine, Family Medicine, or Geriatrics.
• Active, unrestricted state medical license and current DEA registration.
• Experience in post-acute/long-term care, ventilator care, or behavioral health preferred.
• Proficiency with EMR systems and clinical documentation.
• Ability to collaborate with NPs/APPs, nursing, rehabilitation, and administrative staff.
• Current BLS/ACLS (or willingness to obtain).

Responsibilities

• Provide direct, comprehensive medical care to residents in long-term, subacute/post-acute, ventilator, and behavioral health settings.
• Perform rounding, evaluations, and timely follow-ups to ensure continuity of care.
• Lead and participate in multidisciplinary care team meetings; contribute to individualized care plans.
• Manage chronic and complex conditions; respond promptly to status changes with appropriate therapies.
• Communicate clearly with nursing staff, NPs/APPs, patients, families, and consulting providers.
• Collaborate with facility nursing, rehabilitation teams, leadership, and NP to coordinate and optimize care.
• Discuss goals of care and palliative care options; support patients and families in shared decision-making.
• Promote healthy aging through preventive care and proactive treatment plans.
• Document accurately and promptly in the EMR; maintain strong clinical documentation habits.
• Help develop and implement proactive strategies to reduce avoidable hospital transfers and readmissions.
