Job summary

Newave Care is seeking an Attending Nurse Practitioner to provide comprehensive medical management for residents in long-term care, ventilator units, behavioral health, and subacute rehab. You’ll round regularly, coord interdisciplinary care, and apply evidence-based actions to improve outcomes and reduce hospitalizations.

Requirements: AGNP/GNP/FNP board certification, active NJ NP license, BLS and ACLS; LTC/post-acute experience preferred; strong assessment, EMR proficiency, and collaborative

Qualifications

• Board certification as AGNP, GNP, or FNP.
• Active, unrestricted NP license in NJ.
• BLS and ACLS certifications.
• Prior LTC/subacute rehab/ventilator care experience preferred.
• Strong clinical assessment and diagnostic skills; familiarity with post-acute protocols.
• Excellent communication, documentation, and organizational skills; EMR proficiency.
• Demonstrated ability to collaborate with physicians, nursing, rehab, and admin staff.

Responsibilities

• Provide comprehensive medical management for LTC, ventilator, behavioral health, and subacute settings.
• Conduct regular rounding to ensure continuity of care and follow-ups.
• Manage chronic/complex conditions with appropriate modalities and therapies.
• Prevent avoidable hospitalizations through early intervention and vigilant surveillance.
• Coordinate care plans with nursing, rehab, leadership, and physicians.
• Discuss goals of care and palliative options with patients and families.
• Promote healthy aging and preventive care.
• Document thoroughly in the EMR and maintain high documentation standards.
• Participate actively in care-planning meetings.
