Palms Medical Group (PMG) is a not-for-profit Federally Qualified Health Center serving communities across North Florida. The Licensed Clinical Social Worker will provide behavioral health services including crisis intervention, brief psychotherapy, and care coordination as part of the care team.

PMG emphasizes accessible, patient-centered care with bilingual staff, after-hours coverage, and community outreach.

Qualifications

• Master’s degree in social work from a CSWE‑accredited program.
• Current, valid LCSW license in Florida.
• BLS (Basic Life Support) certification current.

Responsabilités

• Provide behavioral health services to PMG patients including emergency assessment, care coordination, brief psychotherapy, and higher level of care recommendations.
• Promote health maintenance and prevention of disease.
• Foster professional relationships between PMG and external health service providers.
• Advise management on operational problems as defined by the organization chart.
• Maintain medical records per PMG policies.
• Be available after hours for behavioral health emergencies and extended office hours coverage.
• Provide coverage for other extended services approved by the Board.
• Handle patient charges, third‑party reimbursement, and ordering necessary therapy supplies.
• Participate in community service and health promotion programs.
• Engage in QA/QI activities as necessary.
• Provide staff and provider education, peer reviews, and updates to committees on Behavioral Health Integration.
• Maintain required certifications and participate in credentialing and peer reviews.
• Potentially supervise staff as requested by the Chief Medical Officer.
• Perform other duties as requested and review medical policies and protocols.
