Palms Medical Group (PMG) is a not-for-profit FQHC serving North Florida with a broad range of primary care and behavioral health services. The Licensed Clinical Social Worker provides crisis intervention, brief psychotherapy, health management, and coordinated care as part of a multidisciplinary team.

The role emphasizes community engagement, after-hours availability, and adherence to PMG policies, with opportunities for professional development and serving underserved populations.

Qualifications

• Must be organized, self-starter and detail oriented.
• Ability to work independently and as part of a team.
• Willingness to travel and cover other sites.

Responsabilités

• Provide behavioral health services including emergency assessment, care coordination and brief psychotherapy.
• Promote health maintenance and disease prevention.
• Foster relationships with local health providers and institutes.
• Report concerns to management as defined by PMG org chart.
• Maintain medical records per PMG policies.
• Be available after hours to treat or triage behavioral health emergencies.
• Share in coverage for extended PMG hours and services as needed.
• Follow PMG policy on patient charges, third-party reimbursement, and supplies.
• Participate in community service and health promotion programs.
• Engage in QA/QI activities.
• Lead staff education and peer reviews.
• Maintain required certifications and credentialing.
• Supervise staff as requested by CMO with Florida requirements.
• Perform other duties and review medical policies as needed.
• Support behavioral health integration initiatives.
