Job summary

Palms Medical Group in North Florida is seeking a Licensed Clinical Social Worker to deliver behavioral health services, crisis intervention, brief psychotherapy, and care coordination within a collaborative care team.

The role emphasizes community-focused, patient-centered care at a not-for-profit FQHC. Requirements include an MSW (CSWE accredited), FL LCSW license, and BLS; after-hours and travel to other sites may be required.

Qualifications

• Master’s degree in social work (CSWE accredited).
• Current and valid LCSW license in the State of Florida.
• Current BLS (basic life support) certification.
• Follow Safety and OSHA requirements.
• Undergo credentialing, utilization and peer reviews.

Responsibilities

• Provide behavioral health services to PMG patients, including crisis assessment and brief psychotherapy.
• Coordinate care and refer to higher levels of care when needed.
• Promote health maintenance and disease prevention.
• Maintain medical records per PMG policies.
• Assist with extended hours and after-hours triage as required.
• Participate in QA/QI activities and staff education.
