Job summary

Salem Clinic, P.C. seeks a Behavioral Health Consultant to join a collaborative care team that integrates behavioral health with primary care in Salem, Oregon.

You will coordinate care with PCPs, screen and treat common disorders, and support patients through evidence-based brief interventions. The role requires a Master's in Social Work and active LCSW licensure, with experience in ambulatory care settings and Epic electronic health records.

Qualifications

• Master's degree in Social Work or equivalent.
• Current LCSW licensure to practice in Oregon.
• Experience screening, assessing, and treatment planning for common mental health and substance use disorders.
• Working knowledge of differential diagnosis, evidence-based psychosocial treatments, and brief behavioral interventions.

Responsibilities

• Coordinate mental and physical health care with patients' primary care providers.
• Screen and assess patients for common mental health and substance use disorders.
• Provide brief, evidence-based interventions such as motivational interviewing and behavioral activation.
• Educate patients and keep them engaged in follow-up care.
• Monitor treatment response and medication management aspects; adjust plans as needed.
