Qualifications

• Master's degree in Social Work, Psychology, Counseling, or a closely related field.
• Current licensure as a Licensed Clinical Social Worker (LCSW).
• Experience in integrated care setting preferred but not required.
• Strong clinical assessment and intervention skills with brief, solution-focused therapy.

Responsibilities

• Address patients' behavioral health needs within the primary care setting.
• Provide brief, evidence-based interventions, psychoeducation, care coordination and consultation.
• Collaborate with medical providers and support staff to support patient health.
