Job summary

Praxis Health is seeking a licensed clinical social worker to join integrated behavioral health teams in Eugene, Oregon. The role operates in a Primary Care Behavioral Health model, offering brief, collaborative interventions within a primary care Clinic setting.

The ideal candidate holds an LCSW, with experience in integrated care and a patient-centered, team-driven approach. This full-time position includes a comprehensive benefits package and a schedule of Monday–Friday, 8am–5pm or four days

Qualifications

• Master’s degree in Social Work, Psychology, Counseling, or a closely related field.
• Current licensure as a Licensed Clinical Social Worker (LCSW).
• Experience in an integrated care setting preferred but not required.
• Strong clinical assessment and intervention skills with a focus on brief, solution-focused therapy.
• Ability to collaborate with a multidisciplinary team and communicate with primary care providers.
• Comfortable using telehealth technology for remote patient care.
• Flexible and adaptable to changing work environments and patient populations.
• Commitment to culturally competent and trauma-informed care.

Responsibilities

• Provide brief, evidence-based interventions within the primary care setting.
• Coordinate care and consult with medical providers and support staff.
• Deliver psychoeducation and care coordination for behavioral health needs.
• Support patients with a wide range of behavioral, emotional, and physical health concerns.
