Job summary

Salem Clinic, P.C. seeks a Behavioral Health Consultant to join a collaborative care team that integrates behavioral health with primary care in Salem, Oregon.

You will coordinate care with PCPs, screen and treat common disorders, and support patients through evidence-based brief interventions. The role requires a Master's in Social Work and active LCSW licensure, with experience in ambulatory care settings and Epic electronic health records.

Qualifications

• Master's degree in Social Work or equivalent.
• Current LCSW licensure to practice in Oregon.
• Experience screening, assessing, and treatment planning for common mental health and substance use disorders.
• Working knowledge of differential diagnosis, evidence-based psychosocial treatments, and brief behavioral interventions.

Responsibilities

As a Behavioral Health Consultant at Salem Clinic, you'll connect medical and mental health care, helping patients take meaningful steps toward healthier lives. You'll work side by side with primary care providers, mental health consultants, and our Comprehensive Care Coordination Department as a core member of a collaborative care team.

What you'll do

• Partner closely with each patient's primary care provider to coordinate mental and physical health care
• Screen and assess patients for common mental health and substance use disorders
• Provide brief, evidence-based interventions such as motivational interviewing, behavioral activation, and problem-solving treatment
• Educate patients about their conditions and treatment options, and keep them engaged in follow-up care
• Track treatment response in person and by phone, and support medication management by monitoring adherence, side effects, and effectiveness
• Consult with mental health consultants and help adjust treatment plans when patients aren't improving as expected
• Connect patients with outside resources, including specialty mental health care, substance use treatment, housing assistance, and vocational rehabilitation
• Build relapse prevention plans with patients who have met their treatment goals
• Document encounters, progress, and recommendations in Epic and keep the care team updated

What you'll bring

• Master's degree in Social Work and licensure as a Licensed Clinical Social Worker
• Experience screening, assessing, and treatment planning for common mental health and substance use disorders
• Working knowledge of differential diagnosis, evidence-based psychosocial treatments, and brief behavioral interventions
• Basic knowledge of psychopharmacology for common mental health disorders
• Strong collaboration and communication skills, and the ability to build therapeutic relationships with patients in person and by phone
• Comfort with Epic and standard office software (Word, Excel, Outlook)

Preferred: Two years of professional LCSW experience, ideally in an ambulatory care setting.
