Salem Clinic is seeking a Behavioral Health Consultant (LCSW) to join a collaborative care team. The role connects medical and mental health care, coordinating with PCPs and other providers to support patients’ mental and physical health.

The position emphasizes evidence-based counseling, patient engagement, and care coordination in a busy primary care setting across Salem Keizer locations.

Qualifications

• Master’s degree in Social Work required and LCSW license.
• Strong collaboration and teamwork skills.
• Ability to engage patients and build therapeutic relationships.
• Experience with screening for mental health/substance use disorders.
• Familiarity with brief evidence-based psychosocial treatments.

Responsibilities

• Coordinate care with PCP and mental health providers.
• Support mental and physical healthcare of referred patients.
• Screen and assess patients for mental health and substance use disorders.
• Provide patient education on disorders and treatment options.
• Utilize evidence-based counseling techniques and brief interventions.
• Document progress in EHR and communicate with care team.
