Job summary

Strive Health is seeking a Licensed Clinical Social Worker (LCSW) to provide psychotherapy and psychosocial support to residents in skilled nursing facilities. The LCSW will conduct assessments, deliver individual and group therapy, collaborate with the interdisciplinary team (IDT) and ensure regulatory compliance.

Qualifications

• MSW required with active Wisconsin LCSW license.
• Enrollment as Medicare Part B provider preferred.
• Experience with geriatric populations or long-term care preferred.
• Strong interpersonal, documentation, and communication skills.

Responsibilities

• Conduct initial assessments and develop treatment plans.
• Provide individual and group psychotherapy to residents.
• Deliver counseling for depression, anxiety, grief, adjustment, and behavioral health issues.
• Document sessions in compliance with Medicare and facility rules.
• Participate in care plan meetings with IDT and facility staff.
• Support residents and families with illness, placement, and end-of-life issues.
• Collaborate with Strive psychiatric NP and staff for coordinated care.
• Educate staff on psychosocial needs and referral processes.
