Job summary

BrightSpring Health Services in Portland, OR seeks an experienced LCSW to provide counseling that supports individuals’ physical, mental, cognitive and emotional recovery within Rehab Without Walls Neuro Rehabilitation. You will perform comprehensive evaluations, set measurable objectives with the rehab team, and coordinate with families and community resources.

Qualifications

• Master's Degree in Clinical Social Work (LCSW).
• Current Licensed Clinical Social Worker Licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury or acute diagnosis.
• CPR certification required.
• Ability to lift 50 pounds and assist with patient handling as needed.

Responsibilities

• Performs comprehensive evaluations, assessing social, emotional and support needs of patients and families.
• Sets measurable objectives formulated with the rehab team.
• Assists health team in understanding social and emotional factors related to health problems.
• Instructs/counsels patients and families in coping with social and emotional responses to illness.
• Participates in care coordination and identifies community resources to meet patient goals.
• Identifies resources to help patients/families achieve objectives.
