Rehab Without Walls Neuro Rehabilitation, an affiliate of BrightSpring Health Services, seeks an LCSW for Home & Community role in South Portland, ME. This full-time position provides psychosocial support and counseling to patients and families living with brain injury, stroke, or spinal injury in home and community settings.

You will collaborate with PT/OT/SLP and the rehab team, conduct evaluations, develop goals, and help patients navigate emotional adjustment and community resources to

Qualifications

• Master's Degree in Social Work or Marriage and Family Therapy.
• Current Social Work or Marriage and Family Therapist licensure in state(s) of practice.
• Minimum of one year experience in rehabilitation settings with brain/spinal injuries.
• Current BLS/First Aid and CPR certifications.

Responsabilités

• Conduct psychosocial evaluations and develop goals with the rehab team.
• Provide 1:1 counseling and social work interventions for patients and families.
• Assist with emotional adjustment and community reintegration planning.
• Coordinate with community resources and support networks.
• Collaborate with PT/OT/SLP and other team members; monitor progress.
• Document and maintain treatment and billing records per regulations.
