Job summary

Rehab Without Walls Neuro Rehabilitation is seeking an experienced LCSW to provide counseling supporting patients’ physical, mental, cognitive, and emotional adjustment during recovery.

You will deliver therapeutic interventions, coordinate with the rehab team, and guide patients toward personal and independent living goals in home/community settings. This role emphasizes collaboration, autonomy, and professional development.

Qualifications

• Master's Degree in Clinical Social Work (LCSW) is required.
• Current LCSW licensure in the state of practice is required.
• Minimum of one year experience in a rehabilitation setting serving individuals with brain/spinal injuries or acute diagnoses.
• BLS First Aid and CPR certification required.
• Ability to lift 50 pounds; moving/lifting up to 100 pounds with safety procedures.
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods.

Responsibilities

• Performs comprehensive evaluations of patients and families.
• Sets measurable objectives with the rehab team.
• Assists the physician and health team in understanding social and emotional factors.
• Instructs/counsels patients and families on coping with social and emotional aspects of illness.
• Participates in care coordination and identifies patient needs with the team.
• Identifies and connects patients/families to community resources to meet objectives.
