Job summary

Qualifications

• Master’s Degree in Clinical Social Work (LCSW) required.
• Current Licensed Clinical Social Worker Licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals with brain/spinal cord injury or acute diagnosis.
• BLS First Aid and CPR certification required.

Responsibilities

• Performs comprehensive evaluations of patients and families.
• Sets measurable objectives with rehab team.
• Assists health team in understanding social and emotional factors impacting health.
• Instructs patients and families in coping with social and emotional responses.
• Participates in care coordination and resources identification.
• Identifies community resources to achieve objectives.
