Qualifications

• Master's Degree in Clinical Social Work (LCSW).
• Current Licensed Clinical Social Worker Licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis.
• CPR certification required.
• Ability to lift 50 pounds; moving, lifting, or transferring may involve up to 100 pounds following safety procedures.
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods.

Responsibilities

• Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families.
• Sets measurable objectives with the rehab team.
• Assists the physician and health team in understanding social and emotional factors related to health problems.
• Instructs/counsels patients and families in treatment and coping with social and emotional responses.
• Participates in care coordination activities and acts as a resource to health team members.
• Identifies and assists patients and families to utilize community resources to meet objectives.
