Job summary

Rehab Without Walls Neuro Rehabilitation is seeking an experienced LCSW to provide counseling services and support for patients transitioning to home and community settings.

You will conduct evaluations, set goals with the rehab team, and guide patients and families through social and emotional aspects of recovery, coordinating care and utilizing community resources to meet objectives.

Qualifications

• Master's degree in Social Work with LCSW licensure.
• Current state licensure as an LCSW in practice state.
• Minimum 1 year rehab setting experience with brain/spine injury or acute diagnosis.

Responsibilities

• Performs comprehensive evaluations of social, emotional, and support needs.
• Sets measurable objectives with rehab team.
• Assists health team in understanding social and emotional factors affecting health.
• Instructs patients and families in coping with social/emotional responses to illness.
• Participates in care coordination and identifies patient needs with team.
• Identifies resources to help patients reach objectives.
