Job summary

The Providence Center, part of Care New England Health System, seeks an ACT II Latino Team 1C CCBH Case Manager to support clients in reducing psychiatric and substance use symptoms and promoting community functioning.

Responsibilities include assessments, care plans, and linking clients to housing, education, employment, and healthcare. Bilingual Spanish required; associate degree is required with a bachelor’s preferred.

Qualifications

• Associate's Degree required; Bachelor's Degree preferred.
• Experience in Human Services field preferred.
• Must have valid driver’s license, registration, current inspection sticker and insured auto.
• Strong knowledge of community resources and systems, including housing, education, employment, and healthcare.
• Bilingual required (Spanish).

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Provide services to assigned caseload meeting program standards with in-person contacts in office and community settings.
• Conduct comprehensive assessments to identify client needs related to housing, education, employment, financial stability, healthcare, and community resources.
• Develop, present and implement individualized care plans with the care team and clients.
• Connect clients to community resources and social services; provide ongoing monitoring and follow-up.
• Work with the care team to align services with clients' mental health treatment plans.
• Perform crisis intervention and risk management as needed; escalate to QMHPs or Emergency Services.
