Responsibilities

• Communicate changes in patient condition promptly to the care team.
• Develop and update care plans with measurable goals and adjust as needed.
• Coordinate discharge planning and referrals, including palliative care when appropriate.
• Educate patients and families using teach-back methods and patient education materials.
