Job summary

Northwestern Medicine is seeking a Registered Nurse with a BSN to deliver patient-centered care under the Relationship Based Care model, adhering to the Illinois Nurse Practice Act and the organization’s ethics and compliance standards.

Under department leadership, you will perform comprehensive assessments, coordinate care, educate patients and families, participate in discharge planning, and contribute to quality improvement initiatives while maintaining a safe, compassionate care environment.

Qualifications

• Must have BSN and current Illinois RN license.
• ADN is acceptable with commitment to obtain BSN within three years of employment.
• Basic Life Support certification required.

Responsibilities

• Performs comprehensive patient assessments and documents findings.
• Communicates changes in patient condition promptly.
• Develops and updates the plan of care with measurable goals.
• Identifies priorities and documents nursing diagnoses.
• Collaborates with patient, family, and health care team in care planning.
• Evaluates patient response to interventions and adjusts care plan as needed.
• Involves discharge planning and makes appropriate referrals, including palliative care.
• Provides complete discharge instructions and medication reconciliation.
