Job summary

Northwestern Medicine is seeking a Registered Nurse (BSN) to join our patient-centered care team in Illinois. The RN will practice under the Relationship Based Care model, adhering to the Nurse Practice Act, ethical standards, and organizational policies while delivering high-quality care.

Responsibilities include comprehensive patient assessment, care coordination, discharge planning, patient education, and collaboration with families and the care team to ensure safety and positive outcomes.

Qualifications

• BSN required and Illinois RN license
• BLS certification required
• Strong interpersonal skills and ability to collaborate with others
• Strong analytic abilities
• ADN acceptable with commitment to obtain BSN within three years

Responsibilities

• Performs a comprehensive assessment on all assigned patients with complete documentation
• Communicates changes in patient's condition promptly
• Utilizes nursing diagnoses and assessment data to derive outcomes and measurable goals in the plan of care
• Identifies and prioritizes patient needs and documents them
• Collaborates with patient, family, and health-care team in care planning and discharge planning referrals
• Evaluates patient response to nursing interventions and updates the plan of care
• Ensures discharge instructions and medication reconciliation are completed and patient portal information is provided
• Uses teach-back for medication education and patient education materials
