Job summary

Boston Medical Center's Transplant Program seeks a Transplant Coordinator (RN) to support pre- and post-kidney transplant patients in ambulatory clinics, coordinate testing, educate families, and liaise with NEOB and referring hospitals. The role collaborates with nephrologists, surgeons, social workers, and other coordinators.

The position requires 3+ years of clinical experience, MA RN license, and outpatient transplant or dialysis exposure.

Qualifications

• Registered Nurse licensed in Massachusetts by start date.
• Minimum 3 years of clinical experience.
• Outpatient transplant or dialysis experience preferred.
• Ability to work in a fast-paced, regulated environment.
• Excellent communication with patients and families.

Responsibilities

• Provides direction and counseling to Transplant Program patients and their families throughout the transplant process.
• Meets with potential transplant recipients and families to discuss options and procedures.
• Encourages patient participation and supports decisions to reduce anxieties.
• Provides patient teaching prior to discharge and ongoing outpatient instruction.
• Acts as patient advocate within the Transplant Program Multi-Disciplinary Team.
• Develops and maintains a care plan with the Transplant Team.
• Follows pre-transplant evaluation protocols and coordinates tests for candidates.
• Assists with discharge planning and ensures regulatory compliance.
