Job summary

Boston Medical Center is seeking a Transplant Coordinator (RN) to oversee pre- and post-kidney transplant care. You will work with nephrologists, surgeons, social workers and coordinators in ambulatory clinics and occasional inpatient rounds.

The role requires a BSN, MA RN license, and 3+ years clinical experience, with dialysis/transplant experience preferred. You will collaborate across teams to manage waitlists, education, and discharge planning.

Qualifications

• Bachelor of Science in Nursing required.
• Massachusetts RN license required by start date.
• 3+ years clinical experience; dialysis/transplant experience preferred.

Responsibilities

• Provide direction and counseling to transplant patients and families.
• Discuss transplant options with recipients and families.
• Support patient education pre- and post-transplant.
• Coordinate pre-transplant evaluation tests with referring hospitals.
• Communicate plan of care with hospital teams and ensure discharge planning.
