Qualifications

Responsibilities

• Coordinate admissions, transfers, and discharges to support timely transitions of care.
• Serve as primary contact between Kaiser Permanente and Delta View Post Acute for resident care coordination.
• Communicate with Kaiser case managers, physicians, nurses, and other health professionals regarding resident needs and treatment plans.
• Maintain accurate clinical documentation and ensure compliance with policies and regulations.
• Support discharge planning and follow-up services, including documentation and family communication.
