Delta View Post Acute in Antioch, CA is seeking a dedicated Kaiser Liaison – LVN to join our team full-time. This role coordinates patient care between Kaiser Permanente and the facility, overseeing admissions, transfers, and discharge planning to ensure seamless transitions.

The ideal LVN will have strong communication, organization, and clinical documentation skills, and experience with Kaiser care coordination. The position offers a comprehensive benefits package and a stable schedule.

Formación

• Current and active California LVN license.
• Experience in SNF, case management, or discharge planning preferred.
• Experience coordinating care with Kaiser Permanente or similar teams.
• Excellent communication and organizational skills.
• Ability to manage multiple priorities with interdisciplinary team.

Responsabilidades

• Serve as primary liaison between Kaiser Permanente and Delta View Post Acute regarding resident care coordination.
• Coordinate admissions, transfers, and discharges to support timely transitions of care.
• Communicate with Kaiser case managers, physicians, nurses, and other healthcare professionals regarding resident needs and treatment plans.
• Review clinical information and assist with coordinating services to support continuity of care.
• Monitor resident status and communicate changes in condition to nursing staff and providers.
• Assist with discharge planning, including coordination of follow-up services and family communication.
• Maintain accurate, timely, and complete clinical documentation and records.
• Support compliance with facility policies and regulatory requirements.
• Collaborate with Nursing leadership and interdisciplinary team to promote quality resident care.
