Job summary

HousingPlus is seeking an Outreach Intake Coordinator to join the Justice Works program in Brooklyn. You will manage referrals, screening, and intake tasks, representing Justice Works in diverse settings.

The role reports to the Senior Program Director and collaborates with subcontractors and referring partners. You will oversee referrals, admissions, and discharges to meet contractual goals, while ensuring accurate documentation and partnership engagement.

Qualifications

• Outreach intake work experience, preferably working with women experiencing homelessness histories, behavioral health needs, and justice impact history.

Responsibilities

• Review and screen all Justice Works referrals to ensure they meet eligibility requirements of the MOCJ contract and subcontractor program requirements
• In partnership with the Justice Works Senior Program Director, assess needs of referred residents and submit appropriate referrals to subcontractors
• Maintain/update the Justice Works occupancy report and admission/discharge log
• Maintain contact with ATI, public defenders and other agencies concerning referrals and participate in partner meetings
• Manage pending referral wait list
• Participate in supervisory sessions and case reviews with Senior Program Director and staff
• Attend agency-wide meetings and staff training
• Performs other duties as assigned by senior leadership
