Responsibilities

• Assist the Activity Director in planning and conducting activities for residents.
• Help create and maintain monthly activity calendar and attendance records.
• Encourage resident participation in self-directed activities such as hobbies and reading.
• Provide activity materials, including Braille or audiobooks when needed.
• Assist with activity care plans based on assessments and interests.
• Coordinate resident outings and transportation as needed.
• Ensure in-room engagement for bed-bound or isolated residents.
• Communicate effectively with residents, families, staff, and community.
• Support discharge planning and resident assessments as assigned.
