Job summary

Reid Health seeks a Joint Care Navigator for the Main Campus to coordinate pre-operative assessments and post-operative care across the care continuum. This role ensures timely clearances and efficient use of resources while educating patients and families.

The position requires at least 3 years of orthopedic clinical experience, Indiana nursing license or CMA/RMA certification, and BLS certification. You will collaborate with surgeons, PCPs, discharge planners, and rehab services.

Qualifications

• Graduation from accredited nursing program (LPN) or completion of Medical Assisting program.
• 3+ years clinical experience with orthopedic patients.
• Indiana Nursing License or CMA/RMA certification; BLS certification.

Responsibilities

• Assess care transition needs of the patient.
• Develop patient-centered goals with the patient and support system.
• Collaborates with Interdisciplinary Team in development of the transition plan of care.
• Performs pre-operative total joint classes and provide appropriate education.
• Educates patient and their potential caregivers regarding the surgery and recovery process.
• Utilizes effective teaching methods tailored to the patient needs.
• Ensures patients complete preoperative and postoperative questions for quality indicators.
• Liaison with PCP to ensure timely postoperative care and avoid ER visits.
• Provides community education on arthritis and joint replacement.
