Job summary

AHN Sewickley Campus is seeking an experienced orthopedic clinical navigator to coordinate care from pre-operative evaluation through post-acute care. You will initiate contact with patients and families, establish roles, and work with physicians and multidisciplinary teams to plan and execute the patient’s care plan.

Qualifications

• RN with Pennsylvania licensure.
• Two years orthopedic/rehabilitation experience.
• Effective communication and public speaking skills.
• Facilitate change and collaborate with physicians and staff.
• Completion of required clearances within 90 days.

Responsibilities

• Coordinate delivery of orthopedic care from pre-operative evaluation through post-acute care.
• Establish open communication with patients and families about care expectations and roles.
• Build relationships across disciplines to enhance patient experience and outcomes.
• Direct and facilitate patient education and navigation during the surgical episode.
