AHN Sewickley Campus is seeking an experienced orthopedic clinical navigator to coordinate care from pre-operative evaluation through post-acute care. You will initiate contact with patients and families, establish roles, and work with physicians and multidisciplinary teams to plan and execute the patient’s care plan.

Work Hours: M-F business hours.
Coordinates and directs the delivery of care for those with orthopedic surgery interventions, especially total joint replacements. In conjunction with the physicians surgical recommendation, initiates contact with all new patients and significant others to establish an open line of communication and identification of their role as the orthopedic clinical navigator to this patient population. They will coordinate/facilitate delivery of these services from pre-operative evaluation through post-acute care, and is responsible for building relationships among the different disciplines in order to enhance patient/significant others experience. In collaboration with physicians they will identify specific patient care needs and accurately direct the delivery of care in order to maximize a patient’s education, understanding, expectations and outcomes for their episode of care with the ultimate goal of helping to ensure each patient is properly prepared for their surgery physically, mentally and emotionally.
Must be a graduate of a professional school of nursing with current Pennsylvania Licensure. Two years of related orthopedic and rehabilitation experience. Must possess professional and effective communication skills and work compatibly with all customers. Effective public speaking. Must be adaptable and facilitate change. Works well with physicians and ancillary departments. Proficiency in the use and application of personal computers. Successful completion of applicable clearances as outlined in Human Resources policy HR-106 within 90 days of commencing employment.
Bachelors degree in nursing or related field. ACLS/PALS. Orthopedic Certification
