Washington County Behavioral Health Division, within the Department of Health and Human Services, seeks a Jail Care Coordinator (Bilingual Spanish) to support adults in custody with severe mental illness, assess needs, and connect them with community resources.

The role involves collaboration with forensics, jail staff, attorneys, Medicaid/CCO’s, behavioral health, substance use, and medical providers, with potential hybrid work options to reduce recidivism after release.

Competenze

• Master's degree in Counseling, Psychology, Social Work, or related field and 3 years of case management experience.
• QMHP certification or an approved clinical license.
• Background check requirements post-offer and CJIS background checks for continuous employment.

Mansioni

• Meet clients in jail to assess needs and develop care plans.
• Connect clients with community resources to divert further legal involvement when possible.
• Collaborate with forensics, jail staff, attorneys, Medicaid/CCO’s, behavioral health, substance use disorder and medical providers.
• Support clients after release to ensure service connection and reduce recidivism.
• Provide general consultation and education for corrections system partners when needed.
