Job summary

Washington County is seeking a Jail Care Coordinator (Bilingual Spanish preferred) to support adults in custody with severe and persistent mental illness. The role involves meeting clients in jail, assessing needs, developing care plans, and linking them with community resources to reduce future involvement with the legal system.

Work will involve collaboration with forensics, jail staff, attorneys, Medicaid/CCO’s, and other health providers, with occasional community-based follow-up after

Qualifications

• Master's degree in Counseling, Psychology, Social Work or related field required.
• Must be certified as a Qualified Mental Health Professional (QMHP) or hold an approved clinical license.

Responsibilities

• Meet adults in jail to assess needs and develop care plans.
• Connect clients with community resources to divert further legal involvement.
• Collaborate with forensics, jail staff, attorneys, Medicaid/CCOs and other providers.
• Provide post-release support to ensure continued service connection and reduce recidivism.
• Occasionally provide consultation/education to corrections partners.
