Job summary

Alliance Health seeks a Diversion Outreach Specialist to support transitions to community living within the TCL program in North Carolina. The role includes assessing needs, coordinating services, and engaging with providers and community partners to improve health outcomes.

The position is full-time with hybrid options. Travel across Mecklenburg County is required, with occasional onsite reporting at the Alliance Office in Charlotte.

Qualifications

• Bachelor's degree in a Human Services field and five years of experience with the population served (SED/SMI or IDD/TBI).
• Or Bachelor's-level RN plus three years of relevant experience working directly with individuals with SMI.
• Or Master’s degree in human services with three years of experience with the population served (SED/SMI or IDD/TBI).
• Master's degree in human services preferred.
• Two years of full-time, post-degree work experience with social service agencies.

Responsibilities

• Assist Diversion Clinician with members transitioning to the community from institutional care settings to community-based care.
• Assist Diversion Clinician in screening referrals in a timely manner.
• Utilize person-centered planning, motivational interviewing and assessments to review information and develop rapport with the members.
• Obtain releases of information to support care management activities.
• Educate members and guardians about rights, options, and provider availability.
• Collaborate with planning team to develop individualized life goals and plans.
• Ensure plans are updated as life circumstances change.
