You will manage ISP timelines, coordinate interdisciplinary teams, ensure timely, person-centered documentation, and support effective use of the EHR system.

This exempt, full-time role offers a competitive salary of $65,000–$70,000 plus strong benefits.

Responsibilities

• ISP Timeline Management: Maintain the agency-wide ISP calendar, monitor deadlines, and initiate preparation at least four weeks prior to meetings.
• Interdisciplinary Team (IDT) Coordination & Engagement: Lead coordination across Residential, Day, Vocational, and Clinical services with timely task completion.
• Documentation & Support Coordinator Communication: Oversee collection and submission of assessments, input from families/guardians, and ISP worksheets.
• Goal Development & Outcome Management: Coordinate development of measurable, person-centered goals across departments.
• EHR System Consistency: Promote standardized documentation and effective use of the electronic health record.
• Quality Assurance & Accountability: Monitor preparation, identify gaps, and support regulatory compliance.
