A social services organization is seeking a Support Coordinator for New Jersey to assist clients in accessing essential services and developing Individualized Service Plans (ISPs). The ideal candidate should possess a Bachelor's degree in a relevant field and have experience working with adults with intellectual and developmental disabilities. This role necessitates excellent organizational and communication skills, along with the ability to travel frequently across assigned counties. The position offers a full-time contract with responsibilities that may include monitoring quality of care and more.

Formación

• 1 year of experience working with the adult I/DD population is essential.
• Ability to travel 50-75% of the time across counties.
• Must have a valid driver's license with no more than 5 points.

Responsabilidades

• Develop and maintain Individualized Service Plans (ISPs).
