Job summary

Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate Long-Term Care Functional Screener to join the IRIS Consulting Team in Eau Claire/Barron Counties, WI. You will administer LTC FS, renewals, and guidance to maintain eligibility while collaborating with ICA team members.

Travel up to 80% is required; a valid LTC FS certification is preferred and bilingual abilities are a plus.

Qualifications

• Bachelor’s degree in social work, psychology, human services, nursing, special education, or related field.
• Minimum 1 year of experience working with seniors or individuals with disabilities.
• Valid LTC FS Certification or willingness to obtain.

Responsibilities

• Build trusting relationships with IRIS participants.
• Administer Long-Term Care Functional Screens (LTC FS) annually or as needed.
• Coordinate and document LTC FS renewals; provide guidance to maintain funding eligibility.
• Collaborate with ICA team members and community partners; participate in staff meetings.
