Job summary

Molina Healthcare in Wisconsin is seeking an Eligibility Screener to support the IRIS program. This remote role combines home visits with community-based screening for adults in need of long-term care services.

You will conduct LTC-FS screenings, maintain accurate documentation, and collaborate with IRIS Consultants, physicians, and care providers. A caring, detail-focused professional with healthcare or human services background is ideal, with required LTC-FS certification or ability to obtain

Qualifications

• At least 1 year of experience serving IRIS program target groups or equivalent.
• Must be certified to conduct/administer the adult long-term care functional screens or pass the Wisconsin LTC-FS certification modules within first week of hire.
• Bachelor’s or higher degree in health/human services or nursing.
• Active Wisconsin RN license if nursing graduate.
• Valid driver’s license and reliable transportation.
• Attention to detail and data accuracy.
• Strong organizational and time-management skills.
• Excellent verbal and written communication skills.
• Demonstrated computer and software skills including Microsoft Office.

Responsibilities

• Meets with IRIS participants face to face to complete the screening process.
• Completes the LTC-FS for people in IRIS according to Wisconsin instructions.
• Verifies screen results with IRIS Consultants, Medicaid Personal Care agencies, and physicians/SSA when needed.
• Maintains documentation and adheres to regulations, ensuring timely completion of screens.
• Participates in weekly team meetings, monthly All Screener Meetings, trainings and testing.
• Other duties as assigned by management.
