Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate Long-Term Care Functional Screener to join our IRIS Consulting Team in Eau Claire/Barron Counties, WI. You’ll help determine eligibility for publicly funded long-term care services and support participants to live independently.

This role requires traveling up to 80%, establishing trust with participants, coordinating LTC FS renewals, and documenting outcomes.

Qualifications

• Bachelor’s degree or higher in Hum. Serv. or related field.
• At least 1 year experience with seniors or individuals with disabilities.
• LTC FS Certification or willingness to obtain.
• Bilingual in Spanish, Hmong, Arabic, Russian or similar.
• Commitment to participant autonomy.
• Proficient with smartphones/tablets/computers.

Responsibilities

• Build trusting relationships with IRIS participants.
• Administer LTC FS annually or as needed.
• Coordinate and document LTC FS renewals.
• Guide participants to maintain funding eligibility.
• Collaborate with ICA team members and partners.
• Attend staff meetings and conferences.
• Prepare reports and may testify in hearings.

Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and detail-oriented Long-Term Care Functional Screener to join our GROWING IRIS Consulting Team in Eau Claire/Barron Counties, WI!

In this role, you’ll help individuals enrolled in the IRIS Program (Include, Respect, I Self-Direct) determine their eligibility for publicly funded long-term care services. You’ll be part of a mission-driven team that empowers people to live independently and meaningfully.

What You’ll Do

• Build trusting relationships with IRIS participants
• Administer Long-Term Care Functional Screens (LTC FS) annually or as needed
• Coordinate and document LTC FS renewals
• Provide guidance to help participants maintain funding eligibility
• Collaborate with ICA team members and community partners
• Participate in staff meetings and multidisciplinary conferences
• Prepare reports and may testify in hearings or reviews
• Ensure compliance with federal and state documentation standards
• Strengthen community connections to support participants

Qualifications

• Valid LTC FS Certification (or willingness to obtain)
• Bilingual in Spanish, Hmong, Arabic, Russian, or other languages is a plus!

• Comfortable with smartphones, tablets, and computer systems
• Knowledge of human behavior, social dynamics, and community resources
• Strong organizational and planning skills
• Ability to work independently and collaboratively

Requirements

• Valid driver’s license & reliable vehicle
• Satisfactory driving record (MVR check required)
• Must pass Caregiver Background Check
• Willingness to travel frequently (day trips + occasional overnight)
