Molina Healthcare is seeking an Eligibility Screener to support Wisconsin's IRIS program remotely, visiting participants in their homes and collaborating with IRIS Consultants.

You will conduct LTC-FS screenings, ensure accurate documentation, and coordinate with providers while upholding compassionate, patient-centered service. The role emphasizes flexible work-life balance and meaningful impact on long-term care access.

Qualifications

• At least 1 year experience serving IRIS program target groups.
• Current LTC-FS certification or must pass modules within first week.
• Bachelor's degree in health or human services or nursing degree.
• If nursing graduate, active WI RN license in good standing.
• Valid driver's license and reliable transportation.
• Attention to detail and ability to ensure accuracy of data.
• Strong organizational and time-management skills.
• Strong customer service skills and ability to work in team and independently.
• Able to adapt to changing organizational needs.
• Excellent verbal and written communication skills.
• Proficient with computers and MS Office.
• Travel required 5%.

Responsabilités

• Meets with IRIS participants face to face to complete the screening process.
• Completes the LTC-FS for people in IRIS according to WI instructions.
• Verifies results with IRIS Consultants, Medicaid Personal Care agencies, and physicians/SSA as needed.
• Maintains documentation in compliance with regulations and completes screens timely.
• Maintains screening skills via weekly meetings and trainings.
• Other duties as assigned by management.
