Job summary

Molina Healthcare is seeking an Eligibility Screener to support Wisconsin's IRIS program from a remote home base, visiting participants in their homes and completing LTC-FS screenings with care.

The role requires a health or human services background, certification eligibility or current licensure (RN if applicable), a valid driver's license, strong organizational and communication skills, and proficiency with Microsoft Office; travel is about 5%.

Qualifications

• Experience with IRIS or similar long-term care programs.
• Must obtain/maintain LTC-FS certification within first week of hire.
• Bachelor's degree in health/human services or nursing degree.
• Active Wisconsin RN license if nursing degree.
• Valid driver's license and reliable transportation.
• Strong data accuracy and documentation skills.
• Excellent verbal and written communication.
• Proficient in Microsoft Office.
• Travel approximately 5% of the time.

Responsibilities

• Meets with IRIS participants face to face to complete the screening process.
• Completes the Adult Long Term Care Functional Screen (LTC-FS) for people in IRIS per Wisconsin instructions.
• Verifies screen results with IRIS Consultants, Medicaid agencies, and physicians when needed.
• Maintains high standards of documentation and regulatory compliance; ensures timely completion of screens.
• Participates in weekly team meetings, monthly All Screener Meetings, trainings and testing to maintain screening skills.
• Performs other duties as assigned by management.
