Job summary

Paycom - ATS is seeking an IRIS Consultant in Wisconsin to support participants in self-directed services. You will conduct home visits and calls, coordinate with natural supports and service providers, and ensure documentation and eligibility tasks are completed accurately.

The role requires travel within a three-hour radius, preparation for DHS guidelines, and collaboration with the quality department to improve program outcomes. A bachelor’s degree in a related field is preferred.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree preferred in a related field.
• Four years of social services experience with HS degree; one year with BA preferred.
• Valid driver’s license and vehicle insurance may be required.

Responsibilities

• Completes home visits and phone contacts per DHS guidelines to assist participants with self-directing services.
• Maintains accurate and timely documentation (ISSP, CIR, NOA, etc.) as required by IRIS policies.
• Directs collaboration between participants, supports, providers, and resources to meet life outcomes.
• Provides training to new ICs and supports coverage for others as needed.
• Maintains financial eligibility by tracking Medicaid reviews and submitting paperwork.
• Attends team meetings and required trainings; partners with quality to support program outcomes.
• Other duties as assigned.
