Job Summary

Lutheran Social Services seeks a Bilingual Spanish IRIS Consultant to support adults with long-term care needs in self-directing their services. In this community-based role, you will conduct in‑home and virtual visits, assess needs and goals, and develop individualized IRIS support plans that promote independence and community inclusion. You will educate participants and families on options and resources, coordinate services, and advocate for their preferences while maintaining accurate documentation. Key Responsibilities
  • Completes home visits and phone contacts in accordance with DHS guidelines to assist participants with self‑directing services and to ensure the health and safety of participants.
  • Travels in a three‑hour radius on a regular basis and farther distances between regions when needed to provide coverage for other ICs.
  • Responsible for accurate and timely completion of Individual Support and Services Plans (ISSP), Critical Incident Reports (CIR), Notice of Actions (NOA), and all other required forms, assessments, and documentation as indicated by IRIS program policies and procedures.
  • Directs collaboration between participants, natural supports, service providers, community resources, and others to assist participants in meeting their life outcomes.
  • Provides assistance to other ICs during times of absences and provides training to new ICs as needed.
  • Works effectively with participants in maintaining financial eligibility by keeping record of when Medicaid reviews are due and by assisting participants in completing forms, making copies of verification items, and submitting paperwork to the Income Maintenance (IM) department.
  • Attends and participates in regularly scheduled phone conferences and face‑to‑face team meetings as required by the Regional Supervisor.
  • Attends all DHS and CFI required orientation and trainings.
  • Partners with the quality department to develop appropriate measures in support of program outcomes.
  • Other duties as assigned.
  • Develop and maintain trusted relationships with IRIS participants.
  • Assist participants in creating and updating individualized IRIS Support and Services Plans.
  • Conduct monthly participant contacts and required quarterly in‑person visits.
  • Promote participant choice, self‑direction, independence, and community inclusion.
  • Monitor participant health, safety, and program outcomes.
  • Educate participants about IRIS enrollment requirements, eligibility, and available resources.
  • Collaborate with Aging and Disability Resource Centers (ADRCs), healthcare providers, community organizations, and service providers.
  • Maintain accurate and timely case documentation within required systems.
  • Complete documentation within established compliance standards and timelines.
  • Assist with annual Long‑Term Care Functional Screens when applicable.
  • Work independently while collaborating with an interdisciplinary support team.
Qualifications
  • One of the following:
  • Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Sociology, Special Education, Nursing, Rehabilitation Services, Healthcare Administration, or a related field and at least one year of supervised experience working with older adults or individuals with disabilities.
  • Four (4) or more years of direct experience providing services to older adults or individuals with disabilities.
  • Current IRIS Consultant in good standing with a Wisconsin DHS‑approved exemption.
