Job summary

Family Support Services in Largo, FL is seeking an IPT Facilitation Coordinator to lead the Integrated Practice Team and coordinate wraparound services for at-risk children and families. You will document and track cases, work with CPI case managers, and connect with community resources to support family safety and stability.

The role requires a Bachelor’s Degree with 3+ years of relevant experience, facilitation/mediation skills, and a valid driver’s license. Apply by contacting Human Resources.

Qualifications

• Bachelor’s Degree required; facilitation/mediation cert or Master’s preferred.
• Three years’ experience with at-risk children and/or families; facilitation/mediation preferred.
• Valid driver’s license; background checks and screenings required.

Responsibilities

• Lead IPT by coordinating a core group from agencies to arrange wraparound and strength-based services.
• Maintain IPT records, action documents, schedules, and IPT notes in FSFN.
• Attend Family Preservation Team referral reviews and network with community resources.
• Process referrals for Family Preservation programs such as FAST, STEPS, Safety Management.
• Perform audits and participate in special projects and department requirements.
