Directions for Living is seeking an Intensive Case Manager to support youth and families in the Treatment Works 1.0 program in Largo, FL. The role focuses on providing intensive case management, coordinating mental health and substance use services, and ensuring timely access to community supports in a trauma-informed, culturally competent framework.

The ICM works within an IDT and develops strength-based treatment plans, ensuring annual client care hours and adherence to safety standards.

Qualifications

• At least five years’ experience in human services or child welfare programs with a Master’s degree in social work or related area.
• Master’s degree in social work or related area with at least 2 years’ experience in human services or child welfare programs.
• Certified as a Child Welfare Professional, preferred.
• Must possess a reliable vehicle to drive to field work and maintain a safe driving record, with appropriate auto insurance and annual vehicle safety inspection if transporting clients.
• Must become certified as a Wraparound Facilitator within 3 months of employment.

POSITION SUMMARY:

This is a professional position providing Intensive Case Management services in the Treatment Works (TW 1.0) Program (COP) under the Outpatient Services Division. The TW 1.0 program provides an intensive wraparound approach to engaging and working with youth who have been identified as displaying violence or are identified as at risk of violence, as part of the Pinellas County School system. TW 1.0 ensures that youth and families are stabilized and prepared to remain in or reenter their educational environment. The TW 1.0 program ensures that youth and families have time sensitive access to mental health, substance use treatment and supportive services focused on strengthening youth and families and proactively supports those youth and families at risk of expulsion from school or law enforcement involvement. All TW 1.0 programming utilizes a solution-focused, family centered approach and evidenced based or research supported practices to assist families in meeting needs and goals while preventing further incidents at school or with law enforcement. Treatment Works 1.0 will work with all members of the family with a team-based approach in a supportive, empowering and respectful manner to assist the family with creating safe and stable coping skills and parenting practices to facilitate long-term behavior change that will ensure that youth are safe in their school and home.

The Intensive Case Manager (ICM) will play an integral role of support, linkage, coaching and guidance for youth and families with a mental health or co-occurring mental health & substance abuse concern, and is responsible for guiding the youth and family in treatment/recovery by providing intensive children’s case management services. The ICM will ensure that case management services focus on providing case management interventions, supporting clinical interventions, utilizing natural supports, community supports, and input and resources from other community partners or professional agencies. The ICM functions as a member of an Integrated Decision Team (IDT) consisting of the ICM, a counselor, and an IDT facilitator. In additional to case management interventions, the ICM is responsible for ensuring the youth and family have an individualized treatment plan that is strength based and built collaboratively with each member of the IDT team during the IDT session. The treatment plan, risk level, and case management interventions are developed by the Integrated Decision Team that consists of the family, natural supports, and formal supports that care about and know the youth and family best. Each IDT member, including the youth and family, contributes valuable knowledge, expertise and perspective that informs the treatment plan and clinical response. In addition, the ICM is responsible for reporting and discussing at IDT’s the youth or families progress/lack of progress, barriers, interventions, safety issues, or any other information important in resolving the safety concerns that bring the youth and family into treatment. The ICM ensures services are delivered using standards of trauma informed care principles, and that plans and clinical interventions address issues regarding how trauma impacts family functioning, and that plans and interventions use trauma resolution skills. The ICM ensures that services are Culturally & Linguistically Competent and are tailored to the unique culture of the child and family. The ICM will ensure that intensive case management services occur in the community at a place convenient and comfortable for the youth and family. This is a field position. The ICM is responsible for ensuring an annual client facing client care number of 1410 hours annually. Achieving the minimum required weekly, monthly and annual client facing client care hours is an essential function of this positon.

Qualifications: EDUCATION EXPERIENCE/REQUIREMENTS:

1. At least -five years’ experience with in human services or child welfare programs with a –Master’s degree in social work or a related area. Master’s degree in social work or a related area with at least (2) years’ experience in human services or child welfare programs.
2. Certified as a Child Welfare Professional, preferred.
3. Must possess a reliable vehicle to drive to do field work and maintain a safe driving record, maintain specific automobile insurance coverage as required by the organization and undergo an annual vehicle safety inspection if transporting clients.
4. Must possess or be eligible for targeted case management certification and Medicaid billing provider number.
5. Must become certified as a Wraparound Facilitator in high fidelity Wraparound within 3 months of employment. Preference will be given to individuals certified as a Wraparound Facilitator in high fidelity Wraparound.
